. It is the parents'/guardians' responsibility to provide all documents. The former school(s) should give you all the proper documents at time of check out. Once you have completed the online enrollment process, including uploading all required documents, please contact our office so we can schedule an appointment for you to see a counselor. Documentation Needed for Enrollment
In order to enroll, the following documentation is required
:
1. Transcripts: A copy of the student’s most recent transcript(s).
For summer 9th grade enrollment, please provide the last report card from the forwarding middle school.
2. Withdrawal grades from most recent school, if student is enrolling during the school year.
3. Immunization records: Students must have all required immunizations.
4. Residency verification documents: Provide current copies of two of the following documents
(dated within the past 60 days):
*Utility bill *Escrow papers *Rental agreement *Voter registration *Driver’s license/ State ID
*Pay stub *Property tax *Correspondence with government agency
We do not accept disconnect notices or mobile phone bills.
If applicable, please provide the following documents
:
1. Special Education documents: Special Education staff members will review documents
prior to enrollment to ensure appropriate placement of the student.
a. IEP: Provide a copy of the most recent IEP.
b. Psychological report: Provide a copy of the most recent psychological report.
2. Section 504 Plan: Please provide a current copy of your child’s Section 504 Plan.
3. Language Assessment (ELPAC)
4. Expulsion Documents (if currently expelled from a public school)
5. Medical Orders: Please provide any current medical orders or other district medical forms upon enrollment and before the start of each school year or when there are changes.
6. Birth Certificate: (If student has been enrolled in another California public school, this is not required. Please
provide only for students enrolling from a private school, out of state, or out of the country.)
*Please inform the Guidance Office staff if the registering student is in foster care.
**Any students entering from another country must have all documents translated into English.
If you move or live out of our city/district attendance you must provide us with a permit and a report of attendance & discipline as well as transcripts.
Once all of the above items are received, an enrollment appointment will be scheduled for the student and parent to meet with a guidance counselor. Thank you.

Documentación Necesaria para Inscripción Escolar
Para poder inscribirse, se requieren los siguientes documentos:
1. Expediente Académico: Una copia del certificado de estudios más reciente del estudiante. Para la inscripción de verano del 9º grado, favor de proporcionar el último boletín de calificaciones de la escuela intermedia de reenvío.
2. Calificaciones de retiro de la escuela más reciente (si el estudiante se está inscribiendo durante el año escolar).
3. Registro de vacunas: Los estudiantes deben tener todas las vacunas necesarias.
4. Documentos de verificación de Residencia: Proporcionar copias actuales de 2 de los siguientes documentos (fechados en los últimos 60 días):

Si se aplica, proporcione los siguientes documentos:
1. Documentos de Educación Especial: Miembros del personal de Educación Especial revisarán los documentos antes de la inscripción para asegurar la colocación adecuada del estudiante.
a. IEP: Proporcionar una copia del IEP más reciente.
b. Reporte Psicológico: Proveer una copia del reporte psicológico más reciente.
2. Plan Sección 504: Favor de proporcionar una copia del Plan Sección 504 de su hijo/a.
3. Evaluación de idioma (ELPAC)
4. Documentos de Expulsión (Si el estudiante está actualmente expulsado de una escuela pública)
5. Órdenes médicas: Favor de proporcionar orden médica actual u otras formas médicas del distrito al momento de inscribirse y antes del inicio de cada año escolar o cuando haya cambios.
6. Acta de Nacimiento: (Si el estudiante ha sido inscrito en otra escuela pública de California, esto no es necesario. Favor de proporcionar sólo para estudiantes que se inscriben desde escuela privada, fuera del estado o fuera del país.)
* Favor de informar al personal de la Oficina de Orientación si el estudiante siendo registrado está en cuidado temporal.
** Cualquier estudiante que ingrese de otro país debe tener todos los documentos traducidos al inglés.
Si se muda o vive fuera de nuestra ciudad / distrito de asistencia, debe proporcionarnos un permiso y un informe
de asistencia y disciplina, así como el boletin de calificaciones.
Una vez que se reciban todos los artículos anteriores, se programará una cita de inscripción para que el estudiante y los padres se reúnan con un consejero. Gracias.
